FY26 - FY27 Rate Sheets

When making insurance enrollment decisions, always consult your healthcare provider and ensure they are within the specific plan’s network or click the "Find a Provider" link on the website for each individual carrier. You may also reach out to the insurance carriers directly, via the Member Services listed on the summary of benefits, to ask specific coverage questions under the plans offered by the Town. Mention that you are a "prospective member" and be ready to reference the specific Group Number as specified below. If you need further assistance or clarification, please reach out to your HR Department and we may be able get further information from our Insurance Account Representative on your behalf.

Staff can log into the Cafeteria Advisors Inc. employee site to check your FSA Balance and File claims 24/7: https://cpaemployee.lh1ondemand.com/Login.aspx?ReturnUrl=%2f

If you want to keep your FSA in FY2026, you must re-enroll. Re-enrollment is not automatic. 

For Fiscal Year 2026, the maximum annual Health Care for the Flexible Spending Account has increased to $3,300.

Aflac offers the following benefits that you can sign up for during this open enrollment: Disability, Accident, Hospital, Cancer, Critical Illness, Vision, Child Delivery, PLUS rider benefits, Adult Whole Life, Adult Term Life & Juvenile Whole Life.

To change your current Aflac or enroll in any new Aflac, please contact Milton's Aflac representative for a phone call or an in person meeting between before Friday, May 30. 

 

Aflac 2025 Open Enrollment

 

If you do not think you will be able to see Aflac in-person, JUST KNOW - We can get you information about all things Aflac related via email, text and calls with you, even if you are not at work during the onsite days listed below.  Please feel free to reach out to me in early May for details, so that you have multiple weeks to review and possibly enroll before the Aflac Open Enrollment ends.

We are happy to help with claims, so reach out if you need claim help or have claim questions too.

·        Monday May 5th - Aflac at Fire Dept, starting @ 9:30am  to 12pm(2)

·        Monday May 5th - Aflac at Cemetery from 12:30pm to 1pm

·        Monday May 5th  - Aflac at Police Dept from 2pm to 4:30pm

·        Thursday May 15th - Aflac at Fire Dept, starting @ 9:30am  to 12pm(3)

·        Thursday May 15th- at Town Hall from 1pm to 1:45pm( Baker conf room)

·        Thursday May 15th - Aflac at Police Dept from 2pm to 4:30pm

·        Tuesday May 20th- Aflac at Fire Dept, starting @ 9:30am  to 12pm(4)

·        Tuesday May 20th - Aflac at Highway(DPW) Dept for12pm to 12:30pm

·        Tuesday May 20th - Aflc Library – drop off packets & info

·        Tuesday May 20th - Aflac at Police Dept from 2pm to 4:30pm

·        Thursday May 22nd - Aflac at Fire Dept, starting @ 9:30am  to 12pm(1)

·        Thursday May 22nd - at Town Hall from 1pm to 4pm( Baker conf room)
